The Challenge:
Our client had an entire suite of urological devices he wished to demonstrate using a single model including:
Guidewire– Stiffness evaluation – compare the ability of different guidewires to straighten ureter on the right side vs the left side
Access Sheath– Ability to pass access sheath over a guidewire, advance up to the proximal ureter
Stent/Tumor stent- Ability to insert guidewire up to kidney, place stents over the guidewire, and accurately position coils of the stent in kidney and bladder
Ureteral Balloon– Ability to pass ureteral balloon over the guidewire, advancing half-past opening of the ureter and demonstrate the ability to inflate to maximum diameter regardless of stricture
Nephrostomy Balloon– Practice gaining access into an appropriate calyx (insert needle, place guidewire through the needle, place the balloon over the guidewire, inflate, place sheath over the balloon, remove the balloon, leaving just the sheath)
Ureteral Catheter– Ability to advance over the guidewire, remove guidewire, and inject contrast (i.e. colored water) into kidney
Basket– Ability to insert up to ureter and kidney and retrieve stones
Laser Fiber– Demonstrate the impact of laser fiber on scope deflection
Scopes (rigid cystoscope, semi-rigid and flexible ureteroscope)
Rigid Cystoscope: Be able to insert rigid cystoscope through the urethra into the bladder to practice gaining access with guidewire into the ureteral orifice
Model Design:
Pulse began with the process by learning how to do all those procedures and conceive of an all in one model. We determined what is three-dimensionally represented vs what could be captured with an image. We determined what is better to have opaque and what is better served with transparency. We designed a method to compress the ureter to create an adjustable stricture. We included horizontal slots to support the ureters that facilitate lateral movement for a custom experience.
Model Development:
Pulse MDM worked very closely with our client to define the sizes and proportions of all anatomical structures. Test models were made using several materials for client evaluation.

This urinary tract training model accommodates 9 urology procedures! Our clients show urologists that stiffer wires provide an advantage when gaining access by showing that flexible wires are more likely to buckle.
